You need to register first to see your results and records online!
We now offer patients online access to their medical records. Unfortunately the General Medical Council, the British Medical Association, NHS England and the Medical Protection Society all strongly discourage giving access to under 16 year olds. So we regret that that this service will not be available to them or their parents.
Before you are able to view your records, you must already be registered with 'Patient Access' which allows you to book appointments or order repeat medication.

In order to view your electronic medical records you must download, print and complete the following documents:
Once you have completed and signed the 2 forms please bring them into the surgery with 2 forms of ID (see the Guide for Patients for details of which documents we can accept). Please be aware that the receptionists may not be able to process the forms immediately.
Please note that for security purposes, the person applying for access to their medical records must bring in the documents personally.
We will email you once your request has been approved which may take up to 7 days.
